For artists under 18 years of age.
If you are an artist in the Child or Youth category (6 to 17 years old), this form must be filled out by a legal relative over 18 years of age, representing the minor artist. The artist's representative must authorize the artist's participation and accept the authorization.

Si eres artista de la categoria Infantil o Juvenil(6 a 17 años), este formulario debe ser llenado por un pariente legal mayor de 18 años, que represente al artista menor de edad. El representante del artistas debe autorizar la participacion del artista y aceptar la autorizacion.

If the artist is a minor and does not have a signed authorization from an adult relative, the artist will be disqualified.
